My clinical practice is primarily for adults with suspected or confirmed head and neck cancer, neck lumps, thyroid/parathyroid disease, salivary gland disorders and airway/voice issues. I accept referrals for patients with a confirmed diagnosis of cancer from UK or abroad who require surgical oncology assessment and/or treatment and wish a second opinion outside of the NHS.
Patients presenting with a symptom or sign suspicious for cancer should ideally in the first instance be referred to their appropriate regional NHS service via their GP as a 2-week wait referral.
Should a private opinion be requested, I accept referrals from General Practitioners, Allied Health Professionals or Secondary Care Providers. I also accept self referrals direct from patients.
Please note that I do not consult on paediatric conditions, or adult primary ear or dizziness symptoms and will direct your request to an alternative otolaryngology specialist.
I am recognised by all of the major insurers and happy to treat both insured patients and those who wish to pay themselves for treatment.
If you have private medical cover please contact your insurer prior to your consultation to ensure that you are covered for the cost of the consultation with me (I am fee-assured through all providers). Your insurance provider will usually issue you with a claim or authorisation number. Please provide this to my secretary in advance of your appointment.
Please note, you will often need an initial referral letter, for example from your own GP or insurer specific GP service, for you to be able to claim.

Fee and charges information
Mr. Fleming is recognised by all of the major insurers and charges within their published fee schedules. These are subject to change without notice and some small excess charges may occasionally arise.

Outpatients – New Appointment: Up to £225*
Outpatients – Follow Up Appointment: Up to £150*
Outpatients - remote video consultation: Up to £125*
*Additional charges may be made for joint clinical oncology/surgical appointments, as well as the use of flexible nasoendoscopy. This will be explained to you prior to incurring any charges.
The Hospital/Private clinic may also charge for X-Rays or ultrasound at the time of the appointment
The fees for surgical procedures are based on the complexity of the procedure. You will be advised of the likely “procedure codes” to obtain pre-authorisation prior to surgery. Please confirm with your insurance company the amount that they will reimburse before booking the procedure. Although you will be kept informed of any excess incurred charges before proceeding, any shortfalls or claims declined by your insurer are your responsibility. If no details are provided you will be invoiced directly at self-funding rates.

All my fees are collected through MEDSERV, who are an authorised medical account billing service and handle all billing and transactions for surgical services. No funds are exchanged with or kept on sites with clinical staff.
Clinic cancellation
Whilst we are sympathetic to any unforseen circumstances, non-attendance at scheduled appointments incurs charges for the use of facilities and staff. It may also mean that other people have to wait longer to receive an appointment. If a clinic appointment is cancelled on the day of the appointment, a 50% cancellation fee will be charged. For non-attendance at a booked appointment without contacting the team in advance, the full fee will be charged. Please therefore contact Mr. Fleming’s secretary as soon as possible to reschedule or cancel an appointment, especially as these cancellation fees are often not recoverable from insurance companies without extenuating circumstances.
Fee Arrangements and the Consultant/Patient Contract
For the avoidance of any confusion, it is important to emphasise that the patient has a contract with the consultant which is entirely separate from their contract with their private medical insurer.
Consultants charge fees and private medical insurers pay benefits (which may or may not fully compensate the patient for the consultant’s fees).
The patient is therefore responsible for the medical consultation/procedure and hospital fees but may reclaim all or some of these amounts from their private medical insurer (if they have one). Usually the private hospitals and/or medical billing service will bill the insurer directly and the patient does not have anything further to pay unless there are some specific exclusions or co-payments that they have previously agreed with the insurer. This shortfall must be settled by the patient with the hospital or billing service..
Consultant fees are handled by a different route. Consultants may submit their fees to the patient although in many cases the consultant will bill the insurance company directly. In any event, if there is any shortfall e.g. in the case of an excess agreement with the insurer, then this becomes the responsibility of the patient.
What do I do if I am unable to get satisfactory service from my insurer?
Consultants are not FSA regulated and cannot offer “business advice”. If you have a complaint against your insurer that cannot be resolved directly, you should seek legal advice or ask your broker or HR department for policies accredited through your employer. In many cases patients have complained to the Financial Ombudsman and this is becoming a preferred route for such matters.
